Renewal/Expired Card Applicants

If you have had an Access 2 card at any time in the past, you are a Renewal Card applicant. We recommend renewing your card two months before it expires. Your renewal card will be mailed to you one month before the expiry of your current card, and will be valid once your old card expires. If your card has already expired, that is OK. You are still considered a renewal applicant.

To renew, you have 2 options – paying online or paying via cheque or money order. Paying online is highly recommended – as you will receive your card much faster.

If Paying Online (Credit Card, Visa Debit)

Paying online is the fastest way to renew your card. If paying online for a renewal, you do not need to send in any paperwork.

Simply:

  1. Click the “Pay Online” button.
  2. Choose the 3-year card or 5-year card option by selecting the card quantity you wish to purchase. Please note: only one card is permitted per applicant.
  3. Click the “Add To Cart” button.
  4. Click the “Checkout” button.
  5. The first screen titled “Purchaser Information” requests the Credit Card or Visa Debit cardholder’s information.
  6. Click the “Continue” button.
  7. The next screen titled “Access 2 Card Applicant Details” requests the applicant (person with the permanent disability)’s  information. The purchaser’s information will be automatically input under the Access 2 cardholder’s information on the next screen. If the purchaser and Access 2 cardholder are different, please manually delete and input the cardholder’s name and date of birth on the correct screen. Under “Card Type” please select Renewal Card.
  8. Click the “Continue” button.
  9. For the “Payment Details” page, please enter: the credit cardholder’s name, credit card number, expiry date and CVV Number/Card Security Code.
  10. Click the “Continue” button.
  11. The next page will ask you to review that the information provided is correct. The purchaser’s information will only be shown.
  12. You will receive a payment confirmation code (ESC-#########) when you are finished. A receipt will be emailed to you within 5 minutes.
  13. Your card will be delivered within 4 weeks (if already expired) or within one month of your previous card’s expiry date (if your card has not yet expired).

 

If Paying With a Cheque or Money Order

If you are not paying online, you must fill out sections B and D of the application form and submit them by mail with a cheque or money order. For a renewal, you do not have to submit Section C (health care professional authorization).

Simply:

  1. Click the “Application Form” button to download the application form.
  2. Complete “Section B – Applicant Information” (ensuring to check off the “Renewal/Expired Card” box).
  3. Complete “Section D – Administration Fee Payment”.
  4. Submit your application and payment together by mail. Please see the application form for the mailing address.
  5. Include a self-addressed, stamped return envelope (for delivery of your card).
